Diagnosis Details: Neurologist diagnosed Alzheimer's in 2004 at the VA and confirmed by a second Neurologist 2 weeks later. Now rated medium level patient.

Exelon Patch

We are now in a 2 year study of what is called the "Exelon Patch".   This study is by Novartis and the patch is 18mgs of a drug.  The study will be completed in October of 2009 and a "mini memory" test with a check of vitals is given every 3 months.  The only side affect has been a slight irratation on the back that itches occasionally.  The neurologist (not VA) has checked the back to be sure there is no disturbing  reaction.

Namenda

Namenda has been given to my husband while he was taking Aricept.  He began with 5mgs of Namenda along with the 10mgs of Aricept.  In October or November of 2007 the Namenda was increased to 10mgs twice a day and the Aricept was discontinued.  This particular medication has been effective.  By that I mean, as with most Alzheimer's patients, they seem to level off at various stages of the disease.  For approximately 1-1/2 to 2 years I have watched my husband go through "plateaus".  Approximately 3 plateaus over this time frame with the loss of short-term memory being the most noticeable.  I have also noticed a short temper, but I cannot attribute that to the medication.  Our doctors have indicated the short temper may well be the disease.  We are now maintaining the 10mgs of Namenda and are in a study of the Exelon Patch.

Aricept

My experience with Aricept began in 1993 with my friend's Mother, who has now died.  The drug was given by an organization experimenting with various drugs for Alzheimers.  During the year Edna was provided the drug (we determined she did not receive a placebo) her Alzheimers seemed to be maintained.  When the study was completed Edna was taken off the drug and her memory began to slip quite noticeably.

 

In 2004 my husband was placed on Aricept by the VA.  This is the only drug available through the VA until you cannot pass a "means test" score of 15.  My experience has been good while my husband was taking Aricept.  Namenda was then added to his medication.

Day Care Induction Va Or Not!

As a General step regarding Alzheimer's by SuziD

Today, at 9:00am,  I took my spouse for enrollment in the VA supported Day Care Center, administered by "Interfaith", a non-denominational service group, using the recreation center of St. Clement's Catholic Church.  The following is the course of events:

1.  Meet and talk with the resident nurse.  Her name was Mary and she began with a        review of his medical history (I provided an in-depth, current chronology that included parents, siblings, their dates of death and causes of death; current doctors, with phone numbers; all medications being taken, amount and daily usage, including vitamins and other over-the-counter medication.  Surgerys: when, and why was it necessary. A copy of the social security card, medicare and any other insurance card and Durable Power of Attorney.

The nurse asked more questions relative to cancer, hepatitus, blood transfusions or blood diseases.  She took blood pressure, weight and then proceeded to administer what was termed a "mini memory" test.  I was not allowed to answer or aid in any of the responses to:  What is the date today; what day is it (Mon. Tues. etc); what season are we in now; what level is this building on (street, 2nd floor, basement);  She then said:  I am going to say three words (house, cake, tree) and will have you repeat them later.  Pointing at objects on her person she asked for the identity of her watch and a pen.  She asked that a sentence be written that included both a noun and a verb.  She then asked for the three words.

2.  A representative of "Interfaith" then requested signatures and gave explanations of numerous forms:  Privacy Practices; days and dates of operation; holiday closings; lunch time (today was minestrone soup, flank steak, potatoes au gratin and mixed vegetables); fees (with a 4 hour minimum - $10 - $10.50 an hour with additional fees for bathing, toenail or finger nail clipping, plus) if you are not covered by the VA; billing; acceptable behavior; Publicity release form; release of information forms and Patient's Rights. Who is an alternate if you can't be reached (give 2 people); In case of your death who will pay bills, care for your spouses needs, find another facility? What if you are hospitalized for 5 days? All of the representatives and nursing staff can and will provide necessary assistance and/or guidance, if needed.  Finally, how to sign in and sign out for each visit and where to pick up your name badge.

My husband is a Veteran covered under the contracted price of $15.00 a day.  For those who are not, other insurance(s) may provide relief. Regarding behavior, there is no hugging of anyone and there is no use of any foul language, e.g. damn, B.S. or J.C., to mention a few and my spouse uses them all and will require immediate behavioral modification.  When an Alzheimer's patient cannot recall the exact word it is not uncommon for them to use explitives.  In addition, huggng does not have to be familiar or suggestive.  I was surprised at this restriction.  All centers are, of course, not the same, but I hope this will provide enough information to help each and everyone prepare in advance.

If I answered your question, ignore this e-mail.  I am new at this site.  That said, the VA did what I believe they called "means testing".  The tests are quite simple for you and me, but perhaps not for Alzheimer's patients.  The questions include:  Your name, age, where were you born, what year is it; what month are we in; then they use paper and pencil and ask the patient to draw different pictures (square, diagonal lines, a triangle) then write a sentence.  At the beginning of each tests they will give the patient three words:  clock, apple, tree, as an example. The words don't connect, like dog and tree (dogs do their business on trees). Before the test is finished they will ask for the three words.  A friend of ours had to put blocks of different shapes in a game board.  He got so mad he threw the blocks across the room and walked out, refusing to ever go back again.  Quite typical of a frustrated patient because they know they are messing up, but don't know why for sure.   I hope this gives you some help.  Oh, the VA charges are minimal because my husband is a Veteran of the Korean War.  If your husband is a Vet, use the resource.  Often they will refer to an outside neurologist for other medication.  E-mail me additional info.
